NHS ADHD Medication Service GP Referral Form

FOR CLIENTS AGED 6+ WITH AN EXISTING DIAGNOSIS OF ADHD.

PLEASE READ THE FOLLOWING GUIDANCE BEFORE REFERRING:

When you submit the form you will receive a confirmation email. This includes instructions on how to complete the referral process.

This is to ensure that we only receive referrals from verified NHS sources.

i) The Oakdale ADHD Medication service is not able to prescribe any medications for any conditions apart from ADHD. We do not prescribe sleep medications, such as melatonin.

ii) Clients with a diagnosis of ADHD can be referred to Oakdale’s ADHD Medication Service under the Right to Choose/ NHS Patient Choice framework. Referring GPs must check with their local ICB that they are following relevant local guidelines on NHS Right to Choose / Patient Choice before submitting this referral.

iii) If a client has an ADHD diagnosis provided by Oakdale, they cannot be referred until they have received their copy of the ADHD diagnostic report.

iv) If the clients ADHD diagnostic assessment and report was not provided by Oakdale, we require a copy of the client’s ADHD diagnostic report. Failure to provide this will result in return of the referral. The below standards MUST be evidenced within the report to be considered a ‘good standard’ assessment which meets the NICE guidelines (NG87) and therefore eligible for the medication service. Reports will be reviewed for the following:

1. Diagnosis of ADHD using DSM5 / ICD 10/11 criteria.

2. Assessment completed by a qualified, experienced professional.

3. Details of how symptoms affect the client in different ways e.g. at work, in education, in social or psychological areas of life. Evidence that difficulties happen across different environments.

4. Evidence gathered through interviews and observations (other forms of evidence may be included too). Children and young people under age 18yrs MUST have been seen face to face for at least one aspect of their assessment.

5. Evidence that a full assessment of the person’s needs took place, including any coexisting conditions, social, family, education/work context, physical and mental health. 

 

Please do not refer clients who do not have a report / without a copy of the report / if it is clear the report will not meet the above criteria.
